The short answer
No. On the direct measurements available — punch biopsies, light microscopy, immunohistology — topical retinoids make the living epidermis thicker and increase collagen formation in the dermis underneath it. The belief that they thin skin is, as far as the published record goes, backwards.
That is an unusually clean answer for a skincare question, and it is worth setting out properly, because the myth is persistent, it has a plausible-sounding origin, and it stops people using the best-evidenced over-the-counter anti-aging active there is.
What the histology actually found
Three studies between them settle the direction of the effect.
Epidermal thickness. A double-blind, placebo-controlled study of 0.1% topical tretinoin reported that it “induced epidermal hyperplasia, compaction of the stratum corneum and the deposition of ultrastructurally visible collagen fibrils in the papillary dermis”, with mitoses significantly increased. Hyperplasia means more cells. The living epidermis got thicker, and the dead surface layer got more compact, both at the same time.
Dermal collagen. The New England Journal of Medicine study that established the mechanism biopsied photodamaged forearm skin and sun-protected buttock skin from the same subjects. Collagen I formation was 56% lower in the photodamaged skin. After ten to twelve months of daily 0.1% tretinoin, collagen I formation rose 80%, against a 14% decrease on vehicle. Photoaged skin is the thinned skin; the retinoid partially restored it.
Cosmetic retinol, not just prescription tretinoin. A twelve-week double-blind study in 45 photoaged women escalated a retinol serum through 0.25%, 0.5% and 1.0% against a matched tretinoin escalation. Week-twelve punch biopsies showed newly formed collagen and greater epidermal thickening in the retinol group than in the tretinoin group. That is cosmetic-industry research rather than an independent trial, and we flag it as such — but it measured the same direction on the same endpoint with an over-the-counter-strength product.
So where did the myth come from?
1. Confusion with topical steroids
This is the big one. Prolonged use of potent topical corticosteroids genuinely does thin skin — cutaneous atrophy is a well-documented, clinically important side effect of that drug class. “Strong cream from the doctor thins your skin” is true, it just refers to a completely different family of molecules.
The irony is that a retinoid is one of the few things shown to push back against it. In an eight-week study of twenty psoriasis patients, skin treated with a potent steroid alone showed a 19% reduction in epidermal thickness; skin treated with the same steroid plus 0.1% tretinoin showed a slight 1% increase. The combination did not reduce the steroid’s effectiveness on the plaques. The retinoid was the protective element in that experiment.
2. The first three weeks feel exactly like thinning
Accelerated turnover compacts and partially sheds the stratum corneum — the dead outer layer whose job is to be a barrier. While that is happening, skin can genuinely feel more delicate: shinier, tighter, quicker to sting, quicker to go red. Every one of those sensations is what a person would describe as “thinner”.

3. Visible capillaries
Some people notice fine vessels more clearly after a few months on a retinoid. A more compact, less scattering surface layer transmits what is beneath it more faithfully — the same reason skin looks more luminous, which nobody complains about. If visible redness is persistent and accompanied by flushing or stinging, that is worth investigating as its own problem rather than filing under thinning; our rosacea page covers where that line falls.
4. Over-application
A face being given several times the intended dose every night is a face with a genuinely compromised barrier, and a compromised barrier is fragile, reactive and uncomfortable. That is a real problem with a simple fix, and it is the most common reason someone’s skin actually does feel wrecked. Our dosing guide covers how to check what you are applying.
The caveat that is real
Retinoids increase sun sensitivity. That is not a thinning claim, it is a photosensitivity one, and it produces a practical instruction that is not optional: daily broad-spectrum sunscreen, every morning, whether or not you can see the sun. The AAD’s guidance on retinoid use says so plainly.
There is also a sequencing point. Skin mid-adjustment — flaking, stinging, visibly irritated — is a poor candidate for waxing, strong acids, aggressive exfoliation or a cosmetic procedure, precisely because the surface layer is doing less of its job than usual. What to look for if this worry is what has been stopping you
- A published low strength. A stated 0.1% or 0.2% lets you start at a dose you can reason about, rather than an unpublished one you have to find out about.
- Barrier ingredients in the formula. Ceramides, niacinamide, panthenol and hyaluronic acid all support the layer that is temporarily doing less work. A formula that includes them shortens the uncomfortable phase.
- A low starting frequency. Two nights a week for a month. The published escalation protocols move in months, and nobody has ever damaged their skin by going slowly.
- A sunscreen you will actually wear. Non-negotiable with any retinoid, and the part of the routine that does most of the prevention.
